Project Zomboid is an open-ended zombie-infested sandbox. Players must manage their character's hunger, thirst, fatigue, health, and mental state, while also scavenging for supplies, crafting weapons and fortifications, and avoiding or fighting off zombies. The game puts players in the shoes of a survivor in a post-apocalyptic world overrun by zombies.

The game has both single-player and multiplayer modes, where you can join forces with other survivors or compete against each other in a hostile environment. The gameplay is open-ended, meaning there is no set goal or endpoint, and your success is measured by how long you can survive. However, the entirety of the game also features a dynamic ecosystem where food and resources will run out over time, and zombies will become more aggressive and numerous. In Project Zomboid, all players must scavenge for supplies in abandoned buildings, such as houses, grocery stores, and warehouses.
